If you need to seek care, please select an Anthem PPO provider. This plan provides the flexibility to see any licensed provider, but when you use providers in the designated Anthem PPO network, [Blue Cross PPO (Prudent Buyer – Large Group)], the annual deductible is lower and covered services are reimbursed at a higher level.

If you need to see care, please select an Anthem PPO provider. This plan provides the flexibility to see any licensed provider, but when you use providers in the designated Anthem PPO network, [National PPO (BlueCard PPO)] the annual deductible is lower and covered services are reimbursed at a higher level.

Your prescription benefits are provided through Express Scripts.
