Anthem HDHP Network CA

If you need care, please select an Anthem HDHP [Blue Cross PPO (Prudent Buyer – Large Group)] provider. This plan has a higher annual deductible and a lower premium cost than the other CNB medical plans. It provides the flexibility to see any licensed provider, but covered services are reimbursed at a higher level when you use providers in the designated Anthem network. 

Anthem HDHP Network Outside CA

If you need care, please select an Anthem HDHP [National PPO (BlueCard PPO)] provider. This plan has a higher annual deductible and a lower premium cost than the other CNB medical plans. It provides the flexibility to see any licensed provider, but covered services are reimbursed at a higher level when you use providers in the designated Anthem network. 

Carry your Insurance Identification card with you at all times.
